I agree to enroll my child, ___________________, in the Sunny Corner Cottage Child Care beginning on _________________.
I have read the attached policies and agree to comply with the rules and responsibilities stated in them.
Care will normally begin at __________ o'clock and end at __________ o'clock on the following days of the week: ____________________________________.
The charge for care of my child is _____________________ per day, if in attendance or not.
Payment obligation is based on the hours I agree to use childcare, and not on the actual hours of attendance. Payment is due whether or not my child actually attends those hours.
Termination may happen if our relationship isn't working out. I may not be able to provide what you desire for your child. I will give you a written two-week notice if I will not be providing care for your child. If you are removing your child from my care I must receive a written two-week notice. Payment by the parent is due for the notice period, whether or not the child is brought to the provider for care.
For new enrollments there is a two-week trial period to ensure proper adjustment, during which either party may terminate the contract without notice.
